Types of forms

The following forms and reports must be submitted through your WCB Online account:

Tier 2/Tier 3 Forms

  • Tier 2 Assessment Report

  • Progress Report (Tier 2 and Tier3)

  • Physical Abilities Report (Tier 2 and Tier 3)

  • Tier 2 Discharge Report

  • Assistive Devices Request

Tier 1 Forms

  • Initial Assessment – Form B

  • Progress Report – Form C

  • Physical Abilities – Form E

  • Assistive Devices Request

All online forms linked to a specific claim number, and their attachments, become a permanent part of the claim file.

Please note: Some users may be experiencing difficulties submitting forms because of missing information in mandatory fields. Please ensure to complete all entries marked with an asterisk (*) before submitting the form.

Document names must remain under 260 characters and not include any of the following special characters: ~ " # % & * : < > ? / \ { | }


Send a form

  1. Use Client Search to select the client the form is for

  2. Select Create New Form and choose the form type. Enter the WCB claim number and the client’s health card number. (If you do not have the claim number of health card number for the client, complete the pdf version of the form from our website and attach it to a secure message.)

  3. Fill out all required fields

  4. Select Save Draft if you need to complete the form later or need to get other team members who are providing treatment to complete relevant sections. Any practitioner with a WCB Online account for your clinic location can access a form or report for a client they are treating and add information.

  5. Attach any relevant files by selecting Attachment. Maximum total attachment size is 10 megabytes; the attachment icon will turn red when the size reaches 75% of maximum.

  6. Select Submit only when the form is completed. Some forms require an electronic signature. Select the signature check box if necessary.

Forms cannot be changed, deleted, or edited after they are submitted.


Send additional forms as an attachment

Some forms and reports available on the WCB website (such as Intake Report – Form A and Tier 3 – Discharge Report) can be submitted as an attachment to a secure message. See Send a Secure Message.


Approval of forms/treatment

You will receive a notification email when the WCB case worker enters or changes an approval status or comments on a form you submitted.
