Forms & letters

In the Forms & Letters Section, you can complete and submit WCB forms directly through MyAccount, including:

  • WCB Injury Report

  • Worker Earnings Information

Filling out WCB injury report

Important information about filling out the Injury Report form:

Mandatory fields on each page of the injury report form must be completed before moving on to the next page. When you click Next and not all mandatory fields have been completed, the fields that are missing information will appear in red and with an asterisk. If you need to gather more information, you can save the report and return to it. Incomplete reports are saved for 90 days.

Step 1: Phone Ext Number: The field for the Phone Ext will only accept three (3) digits. If your extension number is longer than three (3) digits, you can add it to the Comments section on the View/Submit Form screen.

Step 1: Date of Birth – If you received this error "Cannot update a worker details at this time. Please try again later." while attempting to submit an Injury Report, you may have missed entering the worker's date of birthday, which is a required field. To verify, users can click on the draft injury report from the list, navigate back to the first page of the report and see if the DOB is missing. If so, they can enter the DOB and then they should be able to submit the Injury Report successfully. This is a known defect and a solution is in the works to ensure users are prompted for all required fields.

Step 3: Medical attention sought and Date of the appointment: The calendar does not allow for future dates beyond two (2) days from the current date. If you need to add a future date, please add it to the Comments section on the View/Submit Form screen.

Step 7: Usual numbers of hours/days worked: The field will not accept decimal points, eg. 7.5 hours or 2.25 days. You can round up this number, eg. 7.5 make 8 hours or 2.25 make 3 days. The field will also not accept a value of zero. You can enter a one (1) in the field. Please add actual amounts in the comments field on the View/Submit Form screen.

Step 8: Number of hours paid on day time/earnings loss began: The field will not accept decimal points, eg. 7.5 hours or 2.25 days. You can round up this number, eg. 7.5 make 8 hours or 2.25 make 3 days. The field will also not accept a value of zero. You can enter a one (1) in the field. Please add actual amounts in the comments field on the View/Submit Form screen.


Submit a form

  1.  Select the form you wish to submit

  2. Fill in the required fields

  3. Select Send 

Please note: Some users may be experiencing difficulties submitting forms because of missing information in mandatory fields. Please ensure to complete all entries marked with an asterisk (*) before submitting the form.

Document names must remain under 260 characters and not include any of the following special characters: ~ " # % & * : < > ? / \ { | }

You can also view letters sent to you from WCB.  Letters and correspondence related to claims as well as related to your WCB account will all be located on the letters tab.  Users will be able to view these letters based on their MyAccount access role.  For example: A user with Claims Only access will be able to view claim related correspondence, however they will not see letters related to the WCB account.
