In the Forms & Letters Section, you can complete and submit WCB forms directly through MyAccount, including:

  • WCB Injury Report

  • Worker Earnings Information

Filling out WCB injury report

Important information about filling out the Injury Report form:

If a claim has been opened before you submit an Injury Report on MyAccount, the system cannot create a PDF for you to print. You are able to view the report on MyAccount, but if you need a paper copy, please send the case worker a secure message with the request.

Required Fields: You can navigate between pages on the injury report without entering information in all the required fields. If you need to gather more information, you can save the report and return to it later. However, all required fields must be filled in prior to submitting the injury report.

Printing: If a claim has been opened before you submit an Injury Report, the system cannot create a PDF for you to print. You are able to view the report on MyAccount, but if you need a paper copy, please send the case worker a secure message with the request.

Phone Ext Number: The field for the Phone Ext will only accept three (3) digits. If your extension number is longer than three (3) digits, you can add it to the Comments section on the View/Submit Form screen.

Health card and SIN Number: A health card or SIN Number are required to submit an injury report. Please note that we will not ask for the SIN number until a claim has been approved for payment. A Health Card number can be used initially.

Injury type: if the type of injury sustained isn’t reflected in the list of options, please select “other” to this field and enter a description in the space provided.

For back sprain or strain injury: Please select “sprain/strain” from the options provided, and not “back injury”. “Back injury” should be selected only in diagnosed back injuries, such as a herniated disk.

Stress claims: For claims related to stress, please select “other” for this field, and enter a description in the space provided.

Was medical attention sought? If the injury resulted in a worker seeking medical attention, or if the injury could result in a worker seeking medical attention, please select “yes” to this field.

Did the worker lose time? If the worker will lose time from work as a result of the injury, even if the time loss has not yet begun, please select “yes” to this field.

Usual numbers of hours/days worked: Only the “Per day” option can be selected at this time. If you choose “Per Week” or “Other”, you may encounter an error message, and your injury report will not be submitted. Until this issue is resolved, choose “Per day”, and enter the usual number of hours according to this selection.

Dominant Hand: Due to a system error, the injury report form is currently defaulting a worker’s dominant hand to “Left" even if “Right” has been selected in the Worker Job Info section. Until this issue is resolved, please indicate the worker’s correct dominant hand in the comments section on the form.

Comments: If there is not enough space for you to include all of your comments on the injury report, please send us a secure message with any additional details. Shortly after you submit the injury report, a claim number will be generated and will appear on the Claims screen in MyAccount. Use this claim number when sending your secure message, and we will add your notes to our files.


Submit a form

  1.  Select the form you wish to submit

  2. Fill in the required fields

  3. Select Send 

Please note: Some users may be experiencing difficulties submitting forms because of missing information in mandatory fields. Please ensure to complete all entries marked with an asterisk (*) before submitting the form.

Document names must remain under 260 characters and not include any of the following special characters: ~ " # % & * : < > ? / \ { | }

You can also view letters sent to you from WCB.  Letters and correspondence related to claims as well as related to your WCB account will all be located on the letters tab.  Users will be able to view these letters based on their MyAccount access role.  For example: A user with Claims Only access will be able to view claim related correspondence, however they will not see letters related to the WCB account.
